Mental Health Skill Building | Health Connect America
Health Connect America’s Mental Health Skill Building program provides individualized training to enable individuals 18 and older with a serious and persistent mental illness. Our program helps them achieve and maintain community stability and independence in the most appropriate, least restrictive environment.
Support provided includes but is not limited to:
- Training and reinforcement of functional skills and appropriate behavior related to the individual’s health and safety
- Monitoring and education regarding mental health, nutrition, and physical condition
- Increased connection with community resources
The hours and types of support our trained mental health professionals provide are personalized and based on the individual’s assessed needs. We are readily available to meet the needs of families across our regions.
