Intensive In-Home Services | Health Connect AMERICA

Health Connect America’s Intensive In-Home Services provide trauma-informed mental health services to at-risk children, adolescents, and families. This program preserves the family unit and maintains the home environment. Our program also establishes and works toward treatment goals that the family has set. Our trained staff connect our clients and their families to community resources.

Every family is one-of-a-kind. Because of this, our program can be tailored to meet the needs of each client’s and their family’s unique situation. Intensive In-Home services can include, but are not limited to:

  • Crisis intervention and treatment
  • Individual and family counseling
  • Communication skills
  • Case management activities
  • Coordination with other services

These services are designed for children and adolescents who are at risk for removal from the home due to behavioral, mental health or substance abuse problems. Additionally, our program may also be used to reunite a family when a child has been in an out-of-home placement. The family preservation services enable the child to return home and successfully remain there.

Intensive In-Home Services are provided three times per week for a minimum of four weeks. Depending upon the severity of the case, the program may extend up to 16 weeks. And don’t worry, if something comes up outside of a session, Health Connect America has direct care staff on call 24/7.
