At-risk youth and their families face serious issues including grief, abandonment, anger management, substance abuse and family conflict. At Health Connect America, our programs are designed to address these issues, to support the preservation of families and to improve the lives of children-at-risk and their families living in Tennessee.
Health Connect America continually monitors, refines and adds to its existing services to adequately meet the needs of children and families-at-risk throughout the state of Tennessee. We currently offer a wide, comprehensive range of services, including:
Additional Service Offerings
- Assistance with Transportation Coordination
- School Advocacy & Support by working with School Administration
- Weekly Updates for referral sources via e-mail, telephone contact, and/or fax.
(Future plans include an Intranet Site with virtual notification of updates.)
- Inter-Agency Coordination
- Other services accepted by TennCare and other private pay insurance such as inhome case management, peer support, CCFT .
