In-Home Family Preservation
Health Connect America offers In-Home Family Preservation services that are short-term, highly intensive, home-based interventions designed to protect, treat and support families with at least one child at imminent risk of being placed in custody. These services may also be used to reunify a family when a child has been in an out-of-home placement. This level of intervention is needed to enable the child to return home and successfully remain there.
Services
Health Connect America offers a wide, comprehensive range of flexible and responsive interventions tailored to the individual child's and family's needs. Our team will accomplish this by performing a strength(s) based, behaviorally specific needs assessment.
Health Connect America provides in-home psychosocial counseling services to children and/or their families who are experiencing assessed dysfunction. Due to their levels of severity, these ongoing individually-based services are necessary to keep the family intact, or to facilitate reunification of the child with the family. Some clients receiving these services have a diagnosed behavioral illness that if left untreated, places the child at risk for abuse, neglect, or state placement.
We offer individual and family counseling sessions including but not limited to issues related to:

Therapeutic Approaches Used
Our highly qualified staff will utilize varied therapeutic models as indicated in assessments. These models include, but are not limited to: Cognitive-Behavioral Therapy (CBT); Rational-Emotive Therapy (RET); and needs-based Client/Family Centered Counseling. Additional interventions will include: written assignments, skill demonstrations, and behavioral modeling.
These interventions may include one-to-one interactions with individual family members or the entire family aimed at:
- strengthening a family’s cohesiveness;
- providing outlets for stress or recreation;
- use of unstructured time for positive activities; and
- role modeling.
Intensity & Length of Service
Families receive an appropriate mix of face-to-face, child and family and community contacts, as determined by client need and treatment planning. Intensive Family Preservation Services are provided 3 times per week for a minimum of four (4) weeks, depending upon the severity of the case, and may extend up to 16 weeks. Therapists are on call 24/7 for crisis management.
Staff Competency
All outpatient Direct Care Professionals providing traditional counseling services will have minimum staffing requirements as a licensed professional or master's level specialist. All specialists will meet guidelines regarding experience and educational requirements. Additionally our Direct Care Professionals will have demonstrated educational competence. Other forms of monitoring to ensure continued quality care include direct performance observations as well as the administration of written examinations and skills testing.
Supervision
All Direct Care Professionals’ client case loads are reviewed weekly in treatment team meetings under direct supervision of a licensed clinical professional. A web-based information management system is used for supervisor monitoring and review of our Direct Care Professionals’ interventions and activities with all clients.
Client Satisfaction
HCA conducts client satisfaction surveys to enlist feedback from consumers to ensure ongoing development of quality interventions and services.
