Offering individual and family counseling sessions, social and coping skills

In-Home Counseling

Health Connect America provides in-home psychosocial counseling services to children and/or their families who are experiencing assessed dysfunction. Due to their levels of severity, these ongoing individually-based services are necessary to keep the family intact, or to facilitate reunification of the child with the family. Some clients receiving these services have a diagnosed behavioral illness that if left untreated, places the child at risk for abuse, neglect, or state placement.

We offer individual and family counseling sessions to include, social and coping skills training, anger management/reduction training, grief counseling and domestic violence counseling. While at the in-home setting, our staff will also provide medication monitoring and compliance as advised by the physician.

Therapeutic Approaches Used
Our highly qualified staff will utilize varied therapeutic models as indicated in assessments. These models include, but are not limited to: Cognitive-Behavioral Therapy (CBT); Rational-Emotive Therapy (RET); and needs-based Client/Family Centered Counseling. Additional interventions will include: written assignments, skill demonstrations, and behavioral modeling.
           
Service Coordination
By use of our web-based information management system, we are able to provide our varied referral sources with timely reports of clients’ and families’ progress. Our team of counselors and supervisors welcome the opportunity to participate in joint staffing sessions with all other professionals involved with the case.

Staff Competency
All staff regardless of educational and credentialing levels must have a minimum of two years direct contact counseling with at risk youth and their families.  Staff will be evaluated as to knowledge and competency in stages of human development; and therapeutic counseling models.

All staff will be trained in anger management techniques with required participation in continuous quality improvement programs focusing on areas of individual and agency strengths and weaknesses.

All caseworkers of Health Connect America assigned to providing in-home counseling services will have as minimum staffing requirements: licensed professional, master's level specialist, or a bachelor's level with at least two years of experience working with children, with all specialists meeting guidelines regarding experience and educational requirements.

Additionally, our caseworkers will have demonstrated educational competence. Other forms of monitoring to ensure continued quality care include direct performance observations as well as the administration of written examinations and skills testing.

Counselor Supervision
A web-based information management system also is used for supervisor monitoring and review of our caseworkers’ interventions and activities with both the client, their family and the referral source. Supervising counselors will make random home visits to the site of services to observe interventions being done and to receive feedback from the clients/family.top of pagetop
