What is your overall clinical approach to addressing the needs of at-risk children and their families?

Our first and foremost priority is providing the best quality care and treatment for children at-risk and their families with the ultimate goal to keep them safe while empowering them to remain together.  Our services are tailored to individual needs of the families, coupled with HealthConnect’s clinically-proven strategies and needs assessments.

HealthConnect America has developed a unique approach to engaging family members to identify problems and explore solutions.  Our case worker initially performs an extensive evaluation, followed by visits, usually taking place in the client’s familiar home setting.  Our caseworkers utilize techniques that foster open interaction and honesty among its clients.  Clients are assigned clearly defined plans and goals with target dates for achievement on a weekly basis.

How often are reports sent to DCS and other professionals working with the children and family?

The results of each week’s visits are reported weekly to the referring case manager by formal correspondence via telephone, facsimile and/or email.   Future plans include the establishment of an Intranet site with virtual notification and updates.
