Our Approach

We offer a fresh, innovative approach to treatment. With each case, our caseworkers at HealthConnect America understand the importance of personalized service, combined with addressing the multiple needs of the family.

We provide face-to-face support, counseling, and coordination with the family, as well as behavioral, emotional and educational support. Ideally, the support and services will be provided in the child’s home, or identified permanency person’s home. We implement the intensity of care required for the child and family to be successfully reunified, or to prevent the removal of children from their homes.

Our HealthConnect America caseworkers provide casework support for any referral Case Manager, seeing the family throughout the support period. Services are flexible and meet the needs and schedules of the child and family.
